Reduced rate programs are available
for Columbia residents who live on or work full-time
on property subject to the CA assessment and whose
household income is within the Columbia Association
guidelines. This form must accompany the registration
form and registration fee. To receive reduced rates,
this application must be approved prior to final payment
and start of program.
If your child will be taking medication
(prescription and/or non-prescription) that needs to
be administered during the camp day or extended care,
this completed "Medication Order Form" must
be given to each camp, and extended care location on
the first day.
Please complete this form and bring
it with you on the first day of each camp session and
give to the camp/extended care director, ONLY if your
child has a health condition or any condition that may
require special care.
This form is to be completed
(one per family) by, applicants who do not live on property
subject to the Columbia Association annual charge, but
who qualify for resident rates by working full-time on,
or owning property subject to the Columbia Association
annual charge.
