REGISTRATION

  • Please complete one registration form (4 pages if downloaded) per child. If you need more forms, please go to the Download Forms section of this website or contact the camp office at 410-715-3165.
  • To register for extended care, please refer to the gray shaded boxes found at the bottom of each section (Preschool Full-Day One Week, Two Week, etc.).
  • Do not forget to sign the Terms and Conditions located on the last page of the registration form.
  • It is required to complete the month/year of the last Tetanus shot on the registration form. Previous records/dates are not kept on file with the Columbia Association.
  • If any part of the immunization record is missing due to religious or medical reasons, please contact the camp office at 410-715-3165 to obtain a Release Application for Exemption from Immunization Requirements.
  • All incomplete registration forms will be returned to the sender causing a delay in your registration.
  • Do NOT send Special Care Forms or Medication Order Forms to the camp office. Those are intended to be brought to the camp site and given to the director or counselor.
  • Registrations sent without the minimum deposit of $100 will be returned.
  • Please refer to the brochure to answer questions about location, directions and what to bring on the first day.

 Returned Check Fee $35
 Change to Registration Fee $10 (each submitted change after the 2 allowed)
Late Pick Up Fee 1-15 minutes $20 16-30 minutes $40
  31-45 minutes $120 46-60 minutes $160

IMPORTANT DATES
January 19 CA Resident and PPP/PP Members registration begins
February 1 Open registration for all
May 1 Open House at Kahler Hall
May 4, 7, 12 and 15 T-Shirt Pick up
May 15 Final payment due; all registration received after May 15 must include full payment. No personal checks will be accepted after May 15.
May 17-21 Registration closed
May 24 Registration reopens
